What is a functional leg length difference?

A functional leg length difference (functional LLD) is when your legs appear to be different lengths because of how your body is positioned and moving — not because the bones themselves are genuinely different lengths. It's usually caused by muscle tightness, joint restriction, foot mechanics or postural imbalance, and it can often be improved by addressing those underlying contributors.

This is different to a structural (or anatomical) leg length difference, where there is a true difference in bone length. Telling the two apart matters, because the management is quite different — and that's where a thorough assessment makes all the difference.

What causes a functional LLD?

Many everyday factors can contribute, including:

  • Work habits — activities that involve standing and twisting repeatedly in one direction.
  • Sport — soccer, football and other one-sided sports where one side of the body is loaded or used differently to the other.
  • Foot mechanics — when one foot rolls in or flattens more than the other, the legs can effectively sit at different heights.
  • Tight joints or muscles through the hips, pelvis or lower limb.

Over time, your feet often start to compensate for the imbalance, so one foot begins to function differently to the other. This uneven loading can eventually lead to pain — frequently in one foot, but sometimes higher up through the knee, hip or lower back. Unstable foot mechanics can also feed into pelvic imbalances, so several components usually need to be assessed together rather than in isolation.

Every one of our podiatrists holds a Bachelor of Podiatry, is trained in Foot Mobilisation Therapy and is a certified Level 1 Running Coach — which means your gait and running mechanics are genuinely analysed, not glossed over.

A functional LLD assessment may include:

  • A detailed gait and running assessment to see how each leg loads and compensates.
  • Joint and muscle screening to identify tightness or restriction contributing to the imbalance.
  • Foot mobilisation therapy and targeted stretches to free up restricted joints and re-balance the area.

In our experience, working on the whole body's alignment from the feet up gives the best long-term results. That's why we work closely with physios and osteopaths to align the whole body and help resolve a functional LLD before moving to orthotic therapy where it's needed. When custom orthotics are indicated, ours are weightbearing 3D scanned and 3D printed in-house as patient-matched left and right devices — so each foot gets exactly what it needs.

Other clinic services such as shockwave therapy and dry needling are also available where clinically appropriate.

When should I seek help?

It's worth booking an assessment if you've noticed persistent one-sided foot, knee, hip or back pain, uneven shoe wear, or a sense that you're "lopsided" when standing or running. The earlier we identify the contributors, the easier the imbalance is to manage.

Not exactly. A functional LLD means your legs appear different lengths due to muscle tightness, joint restriction or foot mechanics, rather than a true difference in bone length. Our assessment helps distinguish a functional difference (which can often be re-balanced) from a structural one, so we can tailor your management.

Will I need orthotics for a functional LLD?
