Gout

Gout is a form of inflammatory arthritis caused by a build-up of uric acid (urate) crystals in a joint, which triggers sudden, intense pain, redness and swelling [1]. In the foot, it most commonly affects the big toe joint — but it can also strike the midfoot, ankle and other joints.

What causes gout in the feet?

Gout develops when there are higher-than-normal levels of uric acid in the blood, which can form sharp crystals inside a joint [1][2]. A range of factors may contribute, including genetics, diet, alcohol intake, dehydration, certain medications, and medical conditions such as kidney disease or high blood pressure. The big toe joint is a frequent site because it is cooler than more central joints, which may encourage crystals to form [2].

What are the symptoms of gout in the feet?

Gout often comes on suddenly — frequently overnight — and a classic "flare" includes:

• Severe pain in the affected joint (often the big toe)
• Redness, swelling and warmth to the touch
• The joint feeling tender, stiff or hard to move
• Skin that may look shiny and feel hot

Some people also feel generally unwell during an attack. Symptoms can ease over days to weeks, but flares may recur and, over time, can affect the joint if left unmanaged [1][2].

How is gout diagnosed?

Gout is usually diagnosed by a doctor through your history, an examination and, where needed, blood tests for uric acid levels or a sample of joint fluid [1]. Diagnosis and medication management of gout sit with your GP or specialist — so we work alongside them, not in place of them.

How does Blueprint Podiatry help with gout?

While medical management of uric acid is led by your doctor, our podiatry team plays an important role in protecting the foot and keeping you moving. Our three registered podiatrists each hold a Bachelor of Podiatry, training in Foot Mobilisation Therapy, and Level 1 Running Coach certification, giving us a strong understanding of how foot mechanics and load affect painful joints.

• Assessing the affected joint and ruling in or out other causes of foot pain
• Footwear guidance to reduce pressure on a flare-prone joint
• Custom orthotics, designed using our weightbearing 3D scanning and 3D printed in-house, to offload and support affected joints where appropriate
• Gait and load assessment to understand how you walk and where stress concentrates
• General foot care to keep skin and nails healthy, especially important if gout coexists with diabetes or circulation concerns
• Liaising with your GP so your management is joined-up

When should you seek help?

See your GP promptly during a sudden, severe flare — particularly with a hot, swollen joint and fever, which needs medical review to exclude infection. For ongoing foot pain, footwear problems or joint protection, our podiatrists are here to help.
